Rates: to be set during initial contact or consultation

 

Insurance:

 

Your health insurance provider or your employee assistance plan may cover my services in full or in part. Please contact your provider to verify how your plan will pay for mental health services.

I suggest using the following questions to help determine your benefits:

​

• Does my health plan include mental health counseling benefits?

• If so what is my deductible and have I met it yet?

• Does my plan limit how many sessions per year? If so, what is the limit?

   

   

Payment:

I accept cash, checks, and all major credit cards as methods of payment.

 

Cancelation Policy:

If you are unable to attend a session, please notify me as soon as possible or at least 24 hours before the session; otherwise, you may be charged the session rate.
